Letter of Intent, Deposit, and Team Contract Spring 2012 Season
By paying a $400 deposit, I, , an authorized representative of , (Team official) (Team Name)
state that we are committed to playing in the CASL men’s soccer league for the 9 game 2012 spring soccer season which starts March 11th, 2012. I understand and agree to all of the terms of this contract as stated below.
I understand that the $400 deposit is non-refundable should we choose not play in the CASL during the 2012 spring soccer season. I understand that if I have paid a $400 deposit, the balance of the fees, $950, will be due by March 20th, 2012. If I choose not to pay a deposit, the entire fee of $1350 is due February 22nd, 2012. Fees are payable to the Colorado Amateur Soccer League, Inc. All league fees will be paid online on the CASL website by credit card or check. We do not accept cash.
Team Officials/Managers are personally financially responsible for league fees. Teams which have not completely paid their league fees by the deadline may have games forfeited, be relegated, or dropped from the league.
Please provide two team Official/Manager contacts that will represent your team. Team Officials/Managers must be able to contact every player in the club in case of field closures or schedule changes. Two complete contacts are mandatory. Do not put yourself twice.

If you need a specific game time or bye, make sure to note the date and time here.
Requests are filled on a best efforts basis. Once the schedule is published and fields are contracted, I am unable to make changes.

Spring 2012 CASL Rules and Policies
By signing this form you agree to follow all rules, bylaws and polices of the CASL, USSSA and US Soccer. Rules and Policies are subject to change. Additional Rules and Policies will be stated on the CASL website and the CASL schedule.
Managers are responsible for educating their players! You must communicate the rules of the game and policies of the league to your players.
All players and managers must have registered for 2011/2012 with CASA. Go to www.soccercasa.com. Players and managers must be registered to the CASL and the correct team.
Playing an illegal player is an automatic forfeit.
NO EXCEPTIONS! No CASA and USSSA Soccer registration, No Play!
Violent tackles with intent to injure, fighting, spitting or abuse of any type will not be tolerated. Teams, team managers, and the players will be held responsible for all player and fan behavior. Extra fines and suspensions will be assessed. The CASL will not tolerate this type of behavior!
Team managers will register and upload a picture with CASA. Personnel not on the team roster may not be on the team's bench. Up to 3 managers per team. Team managers may not enter the field without permission from the referee. Team managers must stay in the professional area which is from the edge of the midfield circle to the edge of the 18 yard line. No fans are allowed on the same side of the field as the players.
Teams MUST be on the opposite sides of the field from fans. You are responsible for your fans. Referees may ask you to move your fans to the opposite side of the field. Referees have to power to enforce these rules and the game may be stopped by the referee if necessary until everyone complies.
Coaches are absolutely responsible for the conduct of their players and their fans.
Teams are also responsible for player’s fines for cards and suspensions. If your player does not pay the fine, the club must pay the fine when the suspension has been served.
You must provide a net and 2 corner flags for each game. (DSGP provides nets and flags).
You must have 2 sets of jerseys; the first team listed in the schedule is home and must change if necessary. Bring two sets of jerseys to every game.
All games are 90 minutes long. Do not allow referees to change length without both teams consent.
Do not bring dogs or alcohol to the fields.
Once the schedule is set, there will be no changes. You must field a team. The CASL will assess fines of $100 for each and any forfeits. You will forfeit every game until the fine is paid. It does not matter why you forfeit. You are responsible for fielding a team. The CASL Forfeit Notice Cutoff Date is Wednesday at 12 noon for the next Sunday's games. If you have a game that you will not be able to field a team for, I must be informed in writing by that Wednesday at noon in order to avoid the forfeit fine. No games will be changed after the final schedule has been published. You agree to any and all games rescheduled in any way against any team. No refunds or credits will be given for games not played for any reason.
Hold harmless Agreement
I hereby release, waive, discharge, covenant not to sue, and hold harmless from any and all liability, the CASL, it's officers, and any personnel from any claims, costs and expenses whatsoever arising out of or related to any loss, damage, or injury, that may be sustained by my organization (team) and players participating in a CASL game, event, or program.
